The Alabama Department of Public Health is conducting a statewide survey about the information and resources Alabama communities need about brain health, Alzheimer’s disease, and other types of dementia. Anyone can take the survey – you don’t have to be a person living with memory problems or caring for someone with dementia. We want your input! Shape Alabama's Brain Health! The Alabama Department of Public Health is gathering information about what information and resources Alabama communities need about brain health, Alzeheimer's disease, and other types of dementia. The information you provide is confidential and will be used to inform our state's public health programs and services. Who can participate? Any adult living in Alabama. You do not have to have dementia or care for someone with dementia. How long will the survey take? About 10 minutes.
